Medical Advocate/Care Manager

Medical advocacy and care management for older adults and for mental health services. Nursing, social worker or the right candidate can be mentored and trained. This position requires traveling throughout Illinois OR Arizona. We provide medical advocacy at hospitals, rehabs and at home. We place clients in communities. We open long term care insurance claims. We appeal Medicare rehab stays. We bring in entitled services into the home. We create care plans. We run errands and take clients to physician appointments. We act as agent under Powers of Attorney and provide guardianship
