Care Manager – San Francisco, CA

Sage Eldercare specializes in high touch, holistic, in-home dementia care. Our team of Care Managers implement high standards of care and provide creative solutions that address clients' physical, social, and emotional wellbeing. We?re growing our team and seek Care Managers with prior experience in adult mental health, who are highly organized, resourceful and excel at empathic communication. Preference for a Masters degree in one of the following: Gerontology, MSW, MFT, OT or Nursing. This position works from home with travel within San Francisco and northern San Mateo County.
